GD2000 Registration Form

Please note that no information is transmitted to GD electronically.
If you do not mail your form in, you are not registered. There is no electronic/online registration as part of this year's system.

Hello, and welcome to the GD2000 registration process. In order to register, you will need to perform the following steps:

1. Fill out this form online.
2. Press the "Make Letter" button at the bottom of the page.
3. Print out the completed registration form (it is the page you will see after pressing the "Make Letter" button at the bottom of the page, and it is created automatically when you submit your information).
4. Send the hard copy of your form with your check to the address below.

   After we have received your registration materials in the mail, you will receive confirmation by email. Physical mail confirmation will be sent only if you request it. If you don't receive email confirmation within a week, please contact the registration chair at the above address.

   Cancellation policy. If you need to cancel a registration, and if we receive written notification by September 1, 2000 then we will refund your registration, minus $50. After September 1, no refunds will be issued.

   Accommodation.You must arrange accommodations directly with the Conference hotel. Reservations should be made by August 11th to receive the conference room rates. Click here for accommodation information.

   Guests are welcome to attend the Welcome Reception, Conference Banquet or lunches on Thursday and Friday, but will require a ticket for each event. You must purchase tickets no later than September 6, 2000. We cannot guarantee that tickets will be available for purchase at the conference. If you have already registered, you may purchase additional tickets by clicking here.
