GD2000 Extra Ticket Order Form
To use this page you must have already received your registration
confirmation by email.
If you have not already registered,
instructions on how to register are available here.
Please only use this form if you have already registered and would
like to purchase additional tickets for the Welcome Reception,
Conference Banquet and/or Thursday/Friday lunches.
In order to request extra tickets, you will need to perform the
following steps:
- You must have your Registration ID # from your registration
confirmation.
You should receive your confirmation within 7
days of us receiving your registration form (which will be some number
of days after you mail it to us!). Note that we cannot process your
registration form until we receive it. No information is transmitted
electronically.
- Fill out this form online.
- Press the "Make Letter" button at the bottom of the page.
- Print out the completed form (it is created automatically when you submit your information).
- Fill in additional information (credit card info, signature, etc.).
- Send it to the address below.

After we have received your ticket request, you will receive
confirmation by email. Physical mail confirmation will be sent only if
you request it. If you don't receive email confirmation within a week,
please contact the registration chair at the above address.
Cancellation policy. Ticket orders may only be canceled if we
receive written notification by September 1, 2000. No refunds
will be issued after that date.
